Teacher Declaration forms are quick and easy to complete.
Teachers need to have their form signed and all of the boxes ticked (an “X” or smiley faces are not acceptable) by their Headteacher. The form should be dated, in full, at least two years after the date that the teacher registered for CLT.
Signatures need to be signed in ink, or alternatively, a scanned electronic signature may be used. Unsigned forms are in-valid and cannot be processed.
Each of the statements on the Declaration Form must be ticked. The purpose of these is to ensure that the Headteacher confirms and declares that the teacher has met all of the criteria for CLT Status. Incomplete forms are in-valid and cannot be processed. All of the criteria must be met in order to achieve CLT status.
Teacher Reference Numbers (formerly known as DfES number) should also be provided in the space given; this is a seven digit number with the first two numbers separated with a slash (/). The correct Teacher Reference Number is essential so that forms can be processed.
The school address needs to be the same as that given on the Assessors Form which is required from each Headteacher or Head of Service.
It is important that the teacher completes all of the other boxes i.e. title of personal / professional reflection and number of standards in advance of handing the Declaration Form to the Headteacher.
